Unclear on how to create custom decals (footprints)

Hi, while in general I'm finding the Pads documentation to be very helpful, one thing I can't find any good info on is creating custom footprints. Currently to make any decals I can't get easily, I use the Pad Stack dialog box, and this covers me for almost every decal I need. But I now need to create the decals as listed below, and I'm not sure how to do it. I'm thinking it might be related to the 'Copper' or 'Copper Cut Out' options in the Decal Editor, but my attempts at using that haven't worked at all well.

 

I've checked all docs/tutorials but it's possible I'm missing something obvious, so if anyone can help or point me to a good step-by-step guide on doing this, it would be appreciated.
