Hetero Type 2 Symbols Get Different Reference Designators

I created two Hetero Type 2 symbols for a dual op-amp.  One symbol for op-amp1 with power pins, the other for op-amp2 without power pins.  When I place them in a schematic and run PCB interface, they get different reference designators -- U1 and U2, rather than both getting U1.

 

I followed the technote documenation and webinar closely, but can't figure this one out.  Can someone take a quick look at my symbols and tell me where I went wrong?
